Preserve the Bag
Preserve the bag only while it remains controllable.
Capsular bag preservation is not a device choice. It is a sequence of decisions about the capsulorhexis, the remaining zonules, the vitreous face and the support needed next.
Before committing
Is the capsulorhexis intact and usable?
How much support is absent—and is the process progressive?
Can the anterior segment remain stable and vitreous-free?
If control is lost, preserving the bag is no longer the primary goal.
Device logic
Match each tool to one job.
A ring, segment and retractor are not stronger versions of the same idea. Each changes a different part of the mechanical problem.
Hooks
TemporaryHold the equator while the cataract is removed.
CTR
RedistributeShare focal stress around an intact capsular equator.
CTS
AnchorCreate permanent focal scleral support.
Fixation
SecureChoose the material, externalisation and burial plan as one system.
Belt loop
RescueSupport a salvageable late IOL–bag complex.
The operative sequence
- 01Diagnose
Map zonular loss and vitreous before adding stress.
- 02Support
Place temporary equatorial support before the bag destabilises.
- 03Remove
Use low-flow phaco and tangential cortical aspiration.
- 04Fix
Redistribute with a CTR; anchor with a CTS or modified ring when required.
- 05Verify
Centre, remove OVD slowly and confirm that vitreous is controlled.
